We are currently accepting new patients from birth through 21 years of age.  We are part of the JeffPlus network and are affiliated with the Main Line Hospitals (Lankenau, Bryn Mawr & Paoli).  We participate with most major insurance companies including Blue Cross Blue Shield, Aetna, Keystone, Personal Choice, Amerihealth, United Healthcare, PHCS and others.  Please call your insurance provider to check our participation. 

Weiser & Associates does not participate with any medical assistance (Medicaid) programs.  These plans include, but are not limited to, Americhoice, Keystone Mercy, Health Partners and the PA Access Card.

Our office DOES participate with the Pennsylvania CHIP program.  If you have a child under 18 years old that is uninsured, CHIP provides free and low cost insurance.  Please visit their website for more information by clicking here.

In order to make your visit to our office more convenient,  we have attached the registration forms so that you are able to print them at home, read them at your leisure and complete them before your first appointment.  Each form is required for every child, including new members of current families.

Newborns

We here at Weiser & Associates know what a wonderful, yet hectic, time it can be when bringing a new baby home.  We would like to offer some helpful advice so that you may avoid some common mistakes that parents of newborns make when it comes to your insurance company.  Taking the following steps will also help to ensure that your visits to our office are uncomplicated by extra paperwork.  See below for these tips:

• Contact your human resources department or member services at your insurance company and inform them that you have a new baby.  Please try to do this within the first few weeks of the baby's life.  Most insurance companies require that this be done within the first 30 days of life.
• Print the three forms listed above and fill them out before your first visit to our office.  This is required even if you have other children that are established patients with us. 
• Bring the mother's insurance card with you to the baby's first visit.  The baby will be covered under the mother's identification number until he/she receives an individual card or through the first 30 days of life.
• Once you receive notification from your insurance company that your new dependent has been added to your insurance, present this information to the office immediately so that proper billing is completed.
• You may receive a separate card with your child's name.  You will need to give this to the front desk so there is a current copy in your child's chart.
• If you have an HMO insurance you will likely need to select a Primary Care Physician (PCP) for your child.  You may do this at the time of enrollment or you may contact member services and inform them that you wish to select our office for your child's PCP.  Failing to do so may result in a denial of your claims.

NOTE:  Our physicians are on call when the office is closed for medical related questions.   Please call (610) 896-8009 and our answering service will contact the physician that is listed for that day.  Please call the office during regular hours for any general questions. 

Administrative Fees

Please see the following list of administrative fees that our office uses to expedite your needs:

School/Camp Forms:  $5.00 per form

Record Transfers:  $20.00 per record

Driver's License Form:  $20.00*

    If there is no physical within 6 months, we are required by law to perform a brief evaluation of your child.  The office charges $20.00 for this visit due to many insurance companies only allowing one well visit per year. 

   *There is no fee if your child has had a physical within the last 6 months. He/she must be present to sign the form in the presence of a physician.  This is required by the Department of Motor Vehicles. 

Returned Check: $12.00

Billing Late Fee:  $10.00

Sunday/Holiday Office Visit:  $35.00
